    On my brother’s report

    Final Diagnosis
    Urinary bladder. Transurethral resection.
    urothelial carcinoma in situ
    area of invasion into lamia propria is seen
    the muscularis propria present in the biopsy shows no evidence of invasion

    Microscopic description

    The transurethral resection of urinary bladder shows multiple mucosal fragments with lining urothelial cells with high grade dysplasia. An area indicative of lamina propria invasion is seen. The muscularis propria present in the biopsies shows no evidence of invasion. Deeper sections and immunostains confirmed the above findings.

    Can anyone tell me what this means in terms of NMIBC or MIBC. How can I tell what grade and if he should consider a neobladder?

    Carcinoma in situ also known as CIS is a high grade bladder. In your brother’s case according to the path report, it has reached the lining of the bladder. This would be NMIBC at this time. Some patients opt for BCG treatments to try and save their bladder, while some opt to have the bladder removed.
    I have no experience with neobladder so can not comment. Prior to having a RC with a bladder diversion many who have this experience will recommend a 2nd opinion at a major cancer centre.

    Has anyone had success with BCG treatments and CIS. If so did CIS come back.

    Hi Mpj,
    I had CIS for 9 years. It kept coming back but it doesn’t in all people. An alkaline PH through diet is good, no smoking of course and continued BCG might work for your brother. I know it’s a tough decision, but as Jack said.. some people just want it out.. and that’s what I’m doing now. Good luck
