I have had low grade tumours since 1992. Last year new tumours started and were high grade. I had them scraped out then BCG treatments last summer and developed Reactive Arthritis and had to discontinue BCG. Tumours returned in the lower ureter and I had that section respected and reattached.

The Uroligist said I can’t resume BCG but GEMCITABINE is being used in London Ontario.

The Oncologist told me they have never used it here in Northern Ontario and said it is not covered by OHIP. He said he wants to see my bladder removed

My question is, has or us anyone been treated with Gemcitabine in Ontario?

    Hi fortitude
Sorry you have had a recurrence of high grade non-muscle invasive bladder cancer. I have no experience with Gemcitabine instillation’s.
Here is a link on some trials from 2012 regarding this chemo instillation.

I wish I could be of more help.

    Hi Fortitude

I had an RC in April 2016 after being diagnosed with an agressive muscle invasive sarcomatoid carcinoma that developed during BCG treatment for non-muscle invasive bladder cancer. Before the surgery I had 4 months of neoadjuvant chemo using a combination of Cisplatin and Gemcitabine. I believe this combination is widely used to treat bladder cancer. It wasn’t instilled, but was injected intravenously.

In my case, the pathology report showed no presence of cancer in the bladder, prostate or lymph nodes in the removed tissue so the chemo seems to have worked quite well. You might want to ask about this option.

Thanks SJay. My oncologist basically told me chemo does not work for bladder cancer and he only uses it for palliative bladder cancer patients. He said the same of BCG! I was under the impression that he couldn’t be bothered. He did offer to refer my elsewhere. I plan to take him up in it.

    Hi fortitude

If your Uro does not believe in giving treatments for non-muscle invasive high grade bladder cancer, I recommend you find another Uro, the top Urologists who see bladder cancer patients daily are located at the major cancer centers, which their are several in Ontario. If you would like assistance just let us know.
    All the best,
    Jack

    Thanks Jack,

My Urologist is fine, the issue is with the Oncologist.
